Pathogenesis of Varicose Vein Arising from Vulvovaginal Region and Pelvis

Abstract
The primary varicosities generally progress downward in the saphenous system. Sometimes, unusual varicose veins with superficial venous reflux in the presence competent saphenous trunks are seen in out patient clinic. The causes were included vulvoperineal varicosity, round ligament varicosity, persistent sciatic vein incompetence, intraosseous perforating vein incompetence, Klippel-Trenaunay syndrome, congenital venous malformation, and portosystemic collateral-related varicose vein. Especially, origins of varicose veins of vulval, perivulval, gluteal and upper thigh regions were not easily recognized with ultrasonography. To establish diagnosis, the communication from the unusual varicose veins in the legs to the variable pelvic veins by pelvic congestion must be shown. The combined use of computed tomographic venography and ultrasonography may be a possible noninvasive method for the diagnosis of lower extremity unusual varicose veins.
